Token bucket skips the ms→seconds conversion

QA's overnight soak test shows customers granted hundreds of requests in a single burst after the bucket has been idle for a while. The bug is in a helper two files away from the failing signal: a unit-conversion function used by the token-bucket refill math treats ms values as if they were already seconds.
